Application Forms for TANF 400-19-20-10

The allowable application forms used to apply for benefits under the TANF Program are:  

  1. SFN 405, "Application for Economic Assistance Programs";

  2. SFN 719, "TANF Request for Benefits”; and

  3. The Electronic Application found on the Department of Human Service Website.

 

Upon receipt of an application, the TANF Eligibility Worker must determine if it is complete and signed, and a face to face interview must be conducted. If any one of the above signed applications is incomplete, the household must either complete the application or sign a Statement of Facts (or Monthly Report as defined below).

Note: If the application submitted is the SFN 719, “Request for Benefits”, a Statement of Facts (or Monthly Report as defined below) is required.
